  Patient  presented in the dental clinic to replace the old restoration at teeth #11, #21 and #22. After removing the old restoration, we see carries and fracture of tooth #22. To have a nice aesthetic result, we need to follow the Root Membrane technique and place 3 implants.   Upper buccal cortical bone augmentation ..

  A 54 years old female patient was treated with a single implant in the position of #15. The natural second right premolar was previously endodontically treated and restored with a metal cast post and metal-ceramic crown 10 years ago (Fig.1). After the extraction of the fractured tooth (Fig.2) the region was cleaned from all inflammatory tissues and remaining cyst remnants and sterilized with ErYg laser (LiteTouch, Light Instruments, Israel). The procedure revealed excessive bone loss in the buccal area of the cortical bone of this region (Fig.3). A 4,5x10 AR implant was placed with a 70 level of stability (measurement with Mega ISQ). Autogenous bone and Bio-OSS graft (Geistlich Pharma AG, Switzerland) were placed and secured with an i-Gen B2-type membrane. Over all materials an Ossix-Plus (15x25) membrane was placed and stitches were applied (Fig.5,6). After 5 months the i-Gen membrane was removed and a healing abutment was placed. The final restoration was placed 3 months later for personal reasons of the patient. 12 months recall appointment revealed an over 3,5 mm bone regenerated area at #15, according to the dimensions of the i-Gen membrane originally used (Fig.7-12).   AnyRidge Implant Maxillar All - on 4 Technique -by Dr. Miguel Stanleywe have had some extreme cases to resolve and 6 months ago this patient came in with two major problems. One was lack of bone, the other a tight budget. We carefully planned all options and decided to do the best we could to give her fixed teeth. We used a traditional approach with delayed loading and an all on four style surgery. A piezo surgical approach was used. We used the THOR for this. The i and a-PRF helped the GBR and tissue healing. Also a for 3 weeks LLLT (low level laser therapy was applied).We used tissue level abutments after healing, and a hybrid restoration. Megagen anyridge implants. Hopefully next year we do the bottom but save the teeth she has. Unfortunately we can't always get what we want.

  My patient had lost all occluion telations in vertical and horizental aspects.After designing,all upper teeth extracted atraumatically and fresh socket implants inserted .In thin ridge ,ridge split &expansion the implants were inserted and GBR with i gen was done .In lower jaw the Anyridge and mini implants were placed.After 4 months the implants were uncovered and healing abutments installed.The prosthetic phase started and final prosthetic were delivered.

  My patient lost her teeth as car accident.the alveolar ridge in this site had deficiency in both horizental and vertical aspect.I tried to augment the ridge with Timesh & zenograft.Unfortunately after 3 months ,Mesh exposure and tissue lost happen.so i decided to place the implants in the residual bone  and GBR .After 3 months later the healing abutments were installed to the implants and ISQ measurements done.For the final prosthesis design ,the suprastructure with single zirconia crowns helped me for reconstruction of the aesthetic part in both pink and white part.

  Case of the dayInteresting case of massive bone loss after traumatic extraction of the central incisor without any procedure for socket preservation/regeneration. First step was to regenerate the bone loss for ideal 3 D positioning of the implant, a minimum invasive approach was performed with GBR technique using a mixture of allogenic- bovine bone and a titanium mesh. A slow resorbable membrane was used to cover the titanium mesh and a connective tissue graft was placed before suturing the site. After 5 months of bone healing, the titanium mesh secured with 3 screws was removed, and an implant was placed with immediate temporization. Now let's wait and see how the soft tissues will heal around my chair-side fabricated temporary crown. I like this challenging case!
